VOLUNTEER SERVICE REPORT FORM
Thank you very much for contributing your time and skills to ADCO. To fully understand the role volunteers play in the organziation's success and to calculate the contributions our volunteers make, we need to keep track of volunteer time and expenses. This is the form we use to do that. There is no limit to the number of forms you can file, so it makes sense to log your volunteer hours as soon as possible after you've done your volunteering for that day. The day you complete the form will be recorded as the day you did your volunteer work.

These forms are cleared at the end of every month, so that we can prepare monthly statistics. If you are volunteering on or after the 28th, please file your report in advance with an estimated number of volunteer hours for the remaining days of the month, or understand that these hours will be recorded as part of the next month's total.
